Acknowledgment of Risks
I, the undersigned, understand and acknowledge that participation in the BMHS Boys Basketball One Day Camp involves physical activities that may include risks of injury, including but not limited to sprains, strains, fractures, or other physical harm. I voluntarily accept these risks on behalf of my child.
Medical Authorization
In the event of an emergency, I authorize the camp staff to seek medical treatment for my child. I understand that I am responsible for all medical expenses incurred in connection with my child’s participation.
Waiver and Release of Liability
In consideration of my child’s participation in the BMHS Boys Basketball One Day Camp, I hereby release and hold harmless Next Level Sports Camps, Inc and its coaches, staff, volunteers, sponsors, and affiliates from any and all claims, demands, actions, or causes of action, arising out of or related to any loss, damage, or injury that may occur during the camp.
Code of Conduct
I agree that my child will adhere to all rules and instructions provided by the camp staff. I understand that failure to comply may result in my child’s dismissal from the camp without a refund.
Photo/Video Release
I give permission for BMHS Boys Basketball One Day Camp to use photos or videos of my child for promotional purposes.
Parent/Guardian Consent
I have read and fully understand the terms of this waiver and release. I acknowledge that I am signing this document voluntarily and that it is legally binding.
